On your CareFor site you have three in built Care Plan templates – Domiciliary Care, End of Life and Dementia Care. However, you may already have your own Care Plan template which you prefer to use. If so you can easily build your template into CareFor – don’t worry you only need to do this once.

Care Plans are created within the system based on a Service type. So, if you have multiple services, you can have multiple Care Plan templates – and an individual client can also be referred for multiple services.

You can view the Service types on your CareFor system under your Settings:

Please note, if you cannot view Services under the Settings menu you will need to contact the Support team.

Clicking the cog will display the current Care Plan fields for that Service type
